The DOT physical is the medical exam the FMCSA requires of commercial drivers to obtain or renew a CDL. At Clínica Hispana Corazón y Vida we do it in Pasadena, without an appointment, in Spanish, and if you pass you leave with the printed medical certificate in the same visit.

What does the DOT physical check?

  • Health history: illnesses, surgeries, medications and habits.
  • Vision: at least 20/40 in each eye, with or without glasses, field of vision and color recognition.
  • Hearing: forced whisper at five feet or audiometry.
  • Blood pressure and pulse.
  • Urinalysis for protein, blood and sugar (not a drug test).
  • General physical exam: heart, lungs, abdomen, spine, limbs and hernias.

How long does the certificate last?

With blood pressure under 140/90 and no conditions to monitor, the usual certificate is 24 months. With pressure between 140-159/90-99 it is issued for 12 months, and with insulin-treated diabetes for up to 12 months, as long as you bring form MCSA-5870 completed by your treating clinician. The examiner explains the length before signing.

What should I bring?

  • Photo ID and driver's license.
  • Glasses or hearing aids, if you use them.
  • A list of medications with doses.
  • CPAP usage report for the last 90 days, if you have sleep apnea.
  • A recent letter from your cardiologist, if you have a heart condition.
  • Form MCSA-5870, if you use insulin.

What is the visit like in Pasadena?

  1. You complete the driver section of the form at the front desk.
  2. Vital signs, vision, hearing and a urine sample are taken.
  3. The examiner reviews your history and performs the physical exam.
  4. If you pass, you receive the printed Medical Examiner's Certificate (MCSA-5876); keep a copy and submit another to Texas DPS.

The visit usually takes 30 to 45 minutes when you bring complete documents.

What if my blood pressure is high that day?

The examiner can repeat the reading after a few minutes of rest. If it stays high, a shorter certificate can be issued while you get it under control; high blood pressure is treated right here and can be rechecked without an appointment.

No. The urinalysis in the DOT physical screens for protein, blood and sugar. The drug test is a separate service ordered by the employer; we also offer it at the clinic.

Yes, if your pressure is controlled. In that case the certificate is usually issued for one year and rechecked at each renewal. Bring your medication or a list with doses.

Keep the original in the vehicle and submit a copy to the Texas Department of Public Safety (DPS) to keep your CDL active. Your employer may ask for another copy.
