Which symptoms suggest a thyroid problem?

  • Overactive thyroid (hyperthyroidism): weight loss, palpitations, tremor, sweating, anxiety, insomnia, diarrhea.
  • A lump or swelling in the neck, with or without trouble swallowing.

How is the test interpreted?

TSH is the first test: most laboratories use an approximate range of 0.4 to 4.0 mIU/L. A high TSH with low T4 indicates hypothyroidism; a very low TSH with high T4, hyperthyroidism. A borderline result is repeated in a few weeks before treating. No fasting is needed.

What is the treatment like?

For hypothyroidism, levothyroxine is taken on an empty stomach every day, and TSH is repeated six to eight weeks after each dose change. Once stable, the check is every six to twelve months. For hyperthyroidism, management depends on the cause and is coordinated with an endocrinologist.

What is the visit like?

  1. Consultation and neck exam.
  2. Blood draw at the clinic.
  3. Result explained with you and, when appropriate, prescription and instructions.
  4. Date of the follow-up to adjust the dose.

Bring previous lab results and, if you already take thyroid medication, its name and dose.

What if it goes untreated?

Untreated hypothyroidism raises cholesterol, worsens depression and, in pregnancy, affects the baby's development. Uncontrolled hyperthyroidism damages the heart and bones. With timely testing and dose adjustment, most patients regain their energy and usual weight.

No. TSH and T4 can be drawn at any time. If you already take levothyroxine, take it as usual and tell us the dose; the provider will consider it when interpreting the result.

Six to eight weeks after each dose change, and every six to twelve months once TSH is stable. During pregnancy the checks are more frequent.
