If you drive a truck, a bus or any commercial vehicle across state lines, federal law requires a valid medical card. What is the DOT physical for?

The DOT physical confirms that a commercial driver can operate safely. It is required by the FMCSA (Federal Motor Carrier Safety Administration) and can only be signed by a medical examiner listed on its National Registry. When you pass, the examiner issues the Medical Examiner's Certificate (form MCSA-5876), which you must carry and submit in copy to the Texas Department of Public Safety (DPS) to keep your CDL active.

What does the examiner check in Pasadena?

The visit usually takes 30 to 45 minutes and always follows the same order:

  1. Health history. You complete the first part of the form: illnesses, surgeries, medications and habits. Accurate answers prevent problems later.
  2. Vision. You need at least 20/40 in each eye and in both together, with or without glasses, plus a 70-degree field of vision on each side and the ability to tell red, green and amber apart.
  3. Hearing. You must hear a forced whisper from five feet away with at least one ear, or pass an audiometry test with an average of 40 decibels or less. Hearing aids are allowed.
  4. Blood pressure and pulse. This is the reading that shortens the most certificates; the ranges are below.
  5. Urinalysis. It screens for protein, blood and sugar as signs of kidney problems or diabetes. It is not a drug test; that is a separate test ordered by your employer.
  6. General physical exam. Heart, lungs, abdomen, spine, limbs, reflexes and hernias.

If you take blood pressure medication and it is controlled, you normally receive a one-year certificate and get rechecked at each renewal.

Can I pass the exam if I have diabetes?

Yes. With diet, exercise or pills, and no complications, the certificate can last up to two years. If you use insulin, since November 2018 you no longer need a federal exemption: your treating clinician completes form MCSA-5870 within 45 days before the exam, you bring it to the visit, and the examiner can certify you for up to one year. Bring your recent glucose log as well.

What should I bring?

  • Photo ID and your driver's license.
  • Glasses, contact lenses or hearing aids, if you use them.
  • A list of your medications with doses and the name of the prescriber.
  • If you use CPAP for sleep apnea, the device's usage report for the last 90 days.
  • If you have a heart condition, a recent letter from your cardiologist.
  • If you use insulin, the completed MCSA-5870 form.

With these papers in hand, most drivers finish in a single visit.

What if I don't pass?

Failing is not always final. The examiner can often issue a shorter certificate while you get your pressure or sugar under control, or ask for a report from your doctor before signing.
