The immigration medical exam is Form I-693, which USCIS requires when applying for permanent residence. At Clínica Hispana Corazón y Vida in Pasadena it is performed by a USCIS-designated physician, in Spanish, and you receive the completed form in a sealed envelope plus a copy for your records.

What does the I-693 exam include?

  • Review of medical history and vaccination record.
  • Complete physical exam, with questions about mental health and substance use.
  • Blood test for tuberculosis (IGRA) from age two; chest X-ray if positive.
  • Syphilis test between ages 18 and 44 and gonorrhea test between 18 and 24.
  • Missing vaccines according to age: MMR, Tdap or Td, polio, hepatitis A and B, varicella, influenza in season and others per CDC instructions.
  • Completion of Form I-693 in its current edition and delivery in a sealed envelope.

The COVID-19 vaccine is no longer required since January 2025.

Who can sign the I-693?

Only a physician with the USCIS Civil Surgeon designation. Before coming, you can verify the clinic in the official USCIS locator. An exam signed by a physician without the designation is not valid.

What should I bring?

  • Passport or photo ID.
  • Vaccination record, in any language.
  • Medical reports for chronic conditions and a list of medications.
  • Your name exactly as it appears on your immigration application.

Without a written record, a vaccine counts as not given and is replaced.

What is the visit like?

  1. Front desk and document review.
  2. Physical exam and lab samples.
  3. Pending vaccines.
  4. Delivery of the sealed form when results are ready; if a second visit is needed for the X-ray or an additional dose, we give you the date.

The first visit usually takes one to two hours.

How is it submitted to USCIS?

Since December 2, 2024 the I-693 must be filed together with Form I-485. If you file on paper, the envelope stays sealed; if you file online, USCIS instructs you to open it and upload the form. Since June 2025 the form is generally valid only for the application it is submitted with. Coordinate the exam date with your attorney.

It depends on how you file the I-485. On paper, the envelope stays sealed. Online, USCIS instructs you to open it and upload the scanned form. If it was opened by mistake, the exam does not need to be repeated.

You can still take the exam. Vaccines without a written record count as not given and are replaced at the clinic according to your age, which may add cost and one more visit.

Since June 11, 2025, USCIS generally treats the I-693 as valid only for the application it is submitted with, and it must be filed together with the I-485. Confirm the timing with your attorney.
