Every school year and every sports season, schools in Pasadena and the area require a physical exam signed by a health professional. At Clínica Hispana Corazón y Vida we do it without an appointment, in Spanish, and complete the form the same day so your child does not miss the enrollment deadline.

What does the school or sports physical include?

  • The child's or teen's health history, with questions for the parent or guardian.
  • Weight, height, blood pressure and pulse.
  • Vision and hearing check.
  • Examination of heart, lungs, abdomen, spine, joints and hernias.
  • Evaluation of history that matters in sports: fainting, chest pain, asthma, previous injuries.
  • Completion and signature of the school, league or UIL form.
  • Recommendations and, if a vaccine is missing, we give it right here.

Which form should I bring?

For school sports in Texas, the UIL pre-participation physical evaluation form; the school or coach provides it and the parent or guardian fills out the first part before the visit. For school or daycare enrollment, the institution's own form. If you do not bring it, we provide a general certificate, but many programs require their format.

What else should I bring?

  • The child's vaccination record.
  • Glasses, if used.
  • A list of medications and allergies.
  • The parent's or guardian's ID; minors must be accompanied.

What is the visit like?

  1. The parent or guardian completes the history at the front desk.
  2. Vital signs, vision and hearing.
  3. Physical exam with the provider.
  4. Form signed the same day, unless something is found that requires studies before clearing the sport.

The visit usually takes 20 to 30 minutes.

What if a problem is found?

A heart murmur, high blood pressure or an unresolved injury does not always rule out sports, but it may require an EKG, lab work or an additional review before signing. We explain it and coordinate what is needed.

How often is it repeated?

Most Texas schools and leagues require the sports physical every year, and the school physical when enrolling in a new campus or moving to a new stage. If your child had an injury or a new diagnosis during the year, a review before returning to competition is advisable even if the form is still valid.
